Submit a Claim - Claim Instructions

If you are an MOE insured under a Washington policy issued in Washington State who was paid for the total loss of your own vehicle(s) under one of your MOE First Party Coverages (Comprehensive, Collision, and UIM) and received a total loss valuation from MOE based upon an “Autosource” estimate between September 1, 2011 – March 15, 2019, you may be able to get a payment from this class action settlement.

The only way to get a payment is to submit a Claim Form. Claim Forms must be submitted online, or by mail postmarked by September 9, 2019. Claim Forms submitted after this deadline will be deemed untimely, and may not be accepted.

If you had two (or more) qualifying accidents and wish to assert a claim for each, you will need to submit a Claim Form for each qualifying accident.


How To File Online:

After clicking the button below, you will be asked to provide the "Unique ID" that was printed on the top right of the Claim Form that you received in the mail. If you did not receive a Claim Form or you have lost yours, please contact the Settlement Administrator for assistance. After providing your Unique ID and contact information you will be asked to verify your accident information and answer some qualifying questions. Make sure you read the form carefully, certify, and submit it no later than September 9, 2019.

How To File by Mail:

As part of your Notice, you were sent a Claim Form. Read the instructions carefully, fill out the form, sign it, then mail it so that it is postmarked by September 9, 2019 to the following address:
